Step - 01
Registration
Register at Affirma or a nearby transgender-affirming clinic
- Preferred name & pronouns
- Confidential and respectful intake
Step - 02
Medical Evaluation
Consult an Endocrinologist
- Medical history and physical examination
- Baseline blood tests
- Discussion of transition goals and fertility options
Step - 03
Mental Health Assessment
Consult a Psychiatrist / Clinical Psychologist
- Confirm gender incongruence (ICD-11)
- Assess readiness and ability to give informed consent
- Emotional and psychological support
Step - 04
Start Hormone Therapy (GAHT)
- Hormone therapy is started as soon as a Mental Health Professional confirms the diagnosis and capacity for informed consent
- Medical evaluation and baseline tests must be completed
- No fixed waiting period
- Hormones are prescribed and monitored by an endocrinologist
Step - 05
Endocrine Monitoring
- Follow-up every 3 months
- Monitor hormone levels, general health, and side effects
Step - 06
Gender-Affirming Surgery (If Desired)
- Considered after at least 6 months of hormone therapy (when recommended)
- Requires two mental health assessments
- Medical fitness and informed consent required
Step - 07
Long-Term Care & Follow-up
Lifelong endocrinology follow-up
- Diabetes, blood pressure, cholesterol
- Bone health
- Mental health
- HIV and other relevant conditions
- Vaccinations (Hepatitis B, HPV)
